Our team has just published a brand-new article exploring the process of interprofessional collaboration (IPC) and the engagement of patients living with chronic illness in a primary care telehealth context.

Did you know that 44% of Canadians aged 20 and over live with at least one chronic illness?
While primary health care is recognized as the entry point into the health system, care pathways for these individuals are becoming increasingly complex. This reality has significant social and economic impacts across the country.
Monica McGraw, a PhD candidate in nursing, collaborated with her co-supervisors, Professors Marie-Eve Poitras, Isabelle Gaboury, and Yves Couturier (Professor, Université de Sherbrooke). Together, through this scoping review, they present how patients living with chronic illnesses describe their engagement in the IPC process within a primary care telehealth setting
